STAKEHOLDER FORMS NEEDS IMMEDIATE ATTENTION
I’ve made multiple attempts to submit information through the stakeholder dashboard and the issue is still not resolved. The form continues to glitch and is preventing successful submission.
This needs to be addressed ASAP. Please investigate and resolve the problem immediately and provide me with an update once it’s fixed. Thank you. Dr. Faith Walters

Possible causes:
Weak/unstable internet connection
Browser cache or mobile browser glitch
User not signed into the correct Google account
Form requires sign-in or limits one response per email
The email/account being used may already be connected to the form/admin system
Required fields may have been missed or entered incorrectly
File upload question issue, if the form includes uploads
Temporary Google Forms loading/submission error
However, it is definitely not a form issue, a test form was successfully submitted this morning.
